Before Your First Appointment
Please complete your new patient forms before arriving whenever possible. Providing your health history, current symptoms, medications, and insurance information in advance allows our team to prepare for your visit and make the check-in process more efficient.
What to Bring
Please bring the following items to your first appointment:
- A valid photo ID
- Your current insurance card
- A list of medications and supplements
- Relevant X-rays, imaging, or medical records
- Referral or claim information, if applicable
- Any questions you would like to discuss
If your visit involves an automobile accident or workplace injury, please also bring any available claim, attorney, employer, or insurance information.
What to Expect During Your First Visit
Your provider will review your health history, discuss your symptoms, and perform an evaluation to better understand your condition. Additional testing or imaging may be recommended when clinically appropriate.
After the evaluation, your provider will explain their findings and discuss a personalized care plan designed around your needs and recovery goals.
